QUESTION
How do I apply for S.S.I (Supplemental Security Income)

ANSWER
Contact the Social Security Administration office by phone at 319-393-9780 or stop by at 3165 Williams Blvd SW.  (Located right by Dupaco Bank)  They will mail you an application.  If you can get copies of your doctor reports to send in with your application, it will help speed up the process.
