Passport applications are accepted during normal business hours on an appointment-only basis. Please call our office at 319-892-5420 or use the button below to schedule an appointment.
Please bring the following items to your appointment:
- One color passport photo (a photo can be taken during your appointment at the Recorder’s Office for a fee of $10.00)
- A certified copy of your birth record (hospital certificates are not accepted); If you need a birth certificate, first request an appointment with vital records
- A photo ID
- Completed application (do not sign)
- Check or Money Order
All applicants must appear in person. Applications must be signed in the presence of a passport acceptance agent.
Both parents are required to appear with a minor under age 16 or a notarized affidavit from absentee parent is required explaining why they are unable to appear, and a copy of their driver’s license - front and back.
*Two payments are collected at the time of passport execution. The table below shows the total amount of both payments.
The Linn County Recorder charges a $35 passport execution fee that can be made payable by check, money order, cash, or credit card.
The remaining amount is for the passport application portion going to Passport Services. This portion must be made payable to the US Department of State and must be made in the form of check or money order.
